Prior to Your Visit
What to bring to your appointment
To make your visit as smooth as possible, please have the following ready before you arrive.
Call us at (412) 369-4000, Option 2, or request an appointment online.
- ✓Photo ID — A valid driver's license or current photo ID is required at every visit.
- ✓Insurance card(s) — Bring all active insurance cards. We bill your insurer directly for office visits, injections, x-rays, and surgical procedures. Co-pays, deductibles, and co-insurances may be collected at time of visit.
- ✓Referral or authorization — If your plan requires a referral from your primary care physician before a specialist visit or surgery, obtain it before your appointment. Without one, you may be financially responsible for those services.
- ✓Imaging films or CD — Bring any x-rays, MRI, CT, or bone scan images related to your injury or condition.
- ✓Prior medical records — Any records from previous treatment for this injury or condition.
- ✓Knee or hip appointment? — Please wear or bring shorts with no metal hardware.
- ✓Shoulder appointment? — Please wear or bring a tank top or sleeveless shirt.
Pre-register before your visit. You can complete your health history through our patient portal (e-Chart) up to seven days before your appointment. After your visit, office summaries are available there as well. Access e-Chart at ehrweb.tristateortho.com/phxportal

Print and complete these forms before your visit to save time at check-in. New patients should bring the Health History Form and HIPAA Notice on their first visit.
